I'm having trouble writing a proper paper, please help?!?



So I'm applying as a mature student to a university, I havent been in school for 3 years, but my application portfolio requires 3 samples of writing. I am entering a fiction peice I wrote, a published peice, but I would like to write 2 more. My choices are: a summary of a longer essay, a research report or proposal, an essay or paper you wrote in high school/post-secondary institution, a piece of writing done for a volunteer organization, aletter to an editor or a public official, an opinion piece, a book review, a journal entry, or any published text. I want to use Bipolar as a topic. I was trying to do an essay or a research report, but I dont know the difference really between the two. Also, I can't think of a good thesis, I dont plan on an arguement sort of essay, but thats the only kind I can recall from high school. What is an example or method to writing an opinion peice? I need some help brainstorming about bipolar disorder. What want to do is explain the condition, the science behind it, maybe add some statistics, mention some famous people with the disorder, and what it means to me and my family. Does that sound good? What else could I use, add or change? Is there a particular order I should do these paragraphs in? Using that, what TYPE of paper would that make it (essay, research, opinion peice etc.) ?? Any other help will be greatly appreicated, thank you so much for your time :)
